What is Housing Stabilization Services (HSS)?

• HSS is a Medical Assistance (MA) benefit program overseen by the Minnesota Department of Human Services (DHS).

• Integrus supports adults facing disabilities, encompassing mental illness and substance use disorder, as well as seniors, in planning for, discovering, transitioning into, and maintaining affordable housing.

• Any adult residing in Minnesota, who has Medical Assistance coverage, is 18 years of age or older, with a documented disability or mental illness & is experiencing housing instability.

How will your housing specialist assist my client?

Some of the areas a housing stabilization specialist will assist a client with include the following:

• Conducting comprehensive client assessments to understand housing needs and barriers.
• Developing personalized housing plans tailored to the client's circumstances.
• Assisting with completing housing applications and required documentation.
• Connecting clients with housing resources, including subsidies and financial assistance.
• Coordinating with landlords and property managers to secure housing.
• Advocating for clients in resolving housing-related disputes and issues.
• Providing education on tenant rights and responsibilities.
• Monitoring and evaluating progress toward housing stability goals.
