Housing Stabilization Services

Housing Stabilization Services, a Minnesota Medical Assistance benefit program, aims to support individuals with disabilities, encompassing mental illness and substance use disorder, as well as seniors, in the process of locating and maintaining suitable housing.

Minnesota advocates for individuals with disabilities to reside, work, and engage in communities they prefer. Overcoming various challenges and obstacles, such as securing housing, managing budgets, navigating relationships with landlords and neighbors, and comprehending lease agreements, can be arduous. The attainment and maintenance of stable, affordable housing are integral to the overall well-being of the people of Minnesota.

Housing Transition Services

Services that assist a person to plan for, find, and move to a home in the community.

Services that supports a person to maintain living in their home in the community.

Housing Consultation Services

Individuals without a waiver, targeted case management or a MSHO/MSC+ care coordinator, but require Housing Stabilization Services, have the option of completing person-centered planning with one of our partnering enrolled housing consultants. A housing consultant will collaborate with individuals to develop a Housing Focused Person-Centered Plan (DHS-7307).
